You should have received this year’s Choctaw Nation Summer EBT card in the mail.  It will be addressed to your oldest child.  Information from the Choctaw Nation about the program can be found here.

You may wish to download the Healthy Together app and link to your account for in-store help and account balances.  After downloading the app and entering your main contact phone number, the Choctaw Nation choice should be available.  Link using your oldest child’s birthdate.
